Is an AC3 track meant to be played on headphones?

Not really — it is a cinema delivery bitstream. An AC3 stream carries a multichannel mix intended to be decoded by an AV receiver and sent to real speakers placed around a room. A phone or a laptop has to fold it down to two channels before it can play at all, and that downmix is often quieter and flatter than a stereo track mastered as stereo. Convert it if the destination is headphones; keep it if the destination is a receiver. AC-3 is the surround track every AV receiver, DVD player and TV decodes natively, which is why it is still the broadcast baseline.
How much quality has already been lost in an AC3 file?

Whatever the original encoder decided to discard, and it cannot be recovered. AC3 compresses by modelling what the ear will not notice — quiet sounds sitting under loud ones, frequencies at the edges of hearing — and permanently removing it. At a healthy bitrate that is inaudible to almost everyone. What it is not is reversible, so converting to a lossless format preserves the current state rather than restoring the original. AC-3 is lossy multichannel audio, normally 5.1 at 384 or 448 kbps, with metadata for dialogue normalisation and downmixing.
Why does everything from a disc come out as AC3?

The video, the audio and the navigation on an optical disc were standardised around this exact payload so that a fixed-function player could decode it with no software at all. Anything ripped from a disc therefore arrives in it, complete with the era's conventions — interlacing, hard size limits and file splitting included. Why is AC3 on so many discs and broadcasts if I have never chosen it?

The format was specified as the mandatory audio for optical disc and digital broadcast standards, so every player, receiver and set-top box has decoded it for decades and the content industry has encoded into it accordingly. That ubiquity in living-room equipment is exactly matched by its absence from browsers and phone apps.
Can an AC3 file carry a 5.1 surround mix?

Yes — the format handles more than two channels properly, so a surround mix keeps its channel layout rather than being folded to stereo on the way in. Whether you hear it as surround depends on the far end: a receiver will decode all the channels, a phone will downmix at playback time regardless. Converting to a stereo-only format makes that fold permanent.
Does it matter that AC3 is over thirty years old?

Age itself is not the problem — several formats from that era are still perfectly good, precisely because everything has had decades to implement them. What matters is what the era assumed: small files, modest processors, no network, and features nobody needed yet. Where those assumptions still fit, the format is fine. Why will an AC3 file not play in my browser?

Browsers decode a short, fixed list of audio formats and AC3 is outside it, so the file downloads instead of playing or the player simply reports an error. Desktop software with a wider codec set opens it without trouble. If the audio has to play on a web page, converting it is the fix — no player configuration will add a decoder the browser does not ship. AC-3 is inefficient by modern standards and is not accepted by browsers, phones or most web players without a downmix to stereo.
